Since the beginning of my adult nursing course, on every arranging I have had, I have came into contact with many patients with enormous term conditions, who require medications to manage their illness. However, many of these patients do not take their medications as they should, and particularly as it is prescribed. In 1993, The World Health Organisation indicated that only 50% of patients maintain treatment recommendations (WHO, 1993), and in 2009, NICE guidelines state that it is thought that amid a third and half of all medicines prescribed for long term conditions are not taken as recommended (NICE,2009). The estimated drug cost of unused or unwanted medication in the NHS is around £100 million annually. The implications of this are both face-to-face (to the patient) and economic, not only with regards to wasted medicines, but costs for decline in the health of the patient (NICE, 2009).
Over many years, the power of the adult nurse has changed greatly and the number of responsibilities has increased, one of the well-nigh dramatic and controversial of these is in relation to prescribed medications (Chapelhow & Crouch, 2009). I witnessed this first hand whilst on a ward spot when an elderly military personnel was admitted with chest pain.



He later claimed that he did not take his medication as he was require to do so, his GTN spray included. The doctor was aware of the fact the gentleman had avoided taking the spray and prescribed him some for his hospital admission. I witnessed the nurse discussing the issue with the patient to find out the reasons as to why he wasnt taking his medication and richly explained the benefits it would have with regards to preventing chest pain, particularly prior to doing anything strenuous or labouring. It is my understanding that as a nurse you are demand to be an educator to the patient, have a sound association of the medications you administer, and should be prepared to discuss and explain these to the patient at any given time.
